How to Request Records

You have the right to access and obtain copies of your chiropractic records at any time.

  • To make records request please submit a written, signed, and dated request to our office. Please make certain that the request includes what records you are seeking including the date range of the records. Make sure to include your full legal name, date of birth, and a phone number or email address so we can verify your identity.

  • You may submit these requests in person at our office at Soulutions Chiropractic Therapies at 1400 Elm Ave Waco, TX 76701 or via email at docdaniel@soulchiro.com.

  • We will process your request in accordance with applicable Texas law and HIPAA regulations. A reasonable, cost-based fee may apply for copying and mailing.

How to Contact Our Licensing Authority

How to File a Consumer Complaint

​If you believe your health information privacy rights have been violated, you may file a complaint with the Texas Health and Human Services Commission (HHSC):​

  • Online: hhs.texas.gov/report-fraud-abuse-neglect

  • Phone: 1-800-458-9858

  • Mail: Health and Human Services Commission, Consumer Rights and Services, P.O. Box 13247, Austin, TX 78711

You may also file a federal complaint with the U.S. Department of Health and Human Services, Office for Civil Rights at hhs.gov/ocr/complaints​. Filing a complaint will not affect your ability to receive care at our office.
